By signing where indicated on this form, you irrevocably agree to be a participant in BB&B Corp’s (the “Company” or “we/us”) “Business Mastery Transformation” program (the “Program”), and that this “Business Mastery Transformation” Participant Agreement (the “Agreement”) automatically becomes a binding contract between you and the Company, and applies to your participation in the Program.

If any version of special payment arrangement has been offered to you by Ted Miller III or by any representative of the Company, you agree that we have the right to collect the full balance owed in the manner stipulated in writing on this form by such representative, and that your default on that payment plan constitutes our right to declare the unpaid total of payments immediately due and payable.

We respect your privacy and must insist that you respect the privacy of fellow Program participants. By signing where indicated, you agree not to violate the publicity or privacy rights of any Program participant. We respect your confidential and proprietary information ideas, plans and trade secrets (collectively, “Confidential Information”) and must insist that you respect the same rights of fellow Program participants and of the Company. By signing below, you agree (1) not to infringe on any Program participants or the Company’s copyright, patent, trademark, trade secret or other intellectual property rights, (2) that any confidential Information shared by Program participants or any representative of the Company is confidential and Proprietary, and belongs solely and exclusively to the Participant who discloses it or to the Company, (3) you agree not to disclose such information to any other person or use it in any manner other than in discussion with other Program participants during Program sessions. By signing, you further agree that (4) all materials and information provided to you by the Company are its confidential and proprietary intellectual property belonging solely and exclusively to the Company, and may only be used by you as authorized by the Company, and (5) the reproduction, distribution and sale of these materials by anyone but the Company is strictly prohibited. Further, by signing where indicated, you agree that, if you violate, or display any likelihood of violating, any of your agreements contained in this paragraph, the Company and/or the other Program participant(s) will be entitled to injunctive relief to prohibit any such violations to protect against the harm of such violations.

We have made every effort to accurately represent the program and its potential. The testimonials and examples of success in this program used are not intended to represent or guarantee that anyone will achieve the same or similar results. Each individual’s success depends on many factors, including his or her background, dedication, desire, action taken, and motivation. By signing where indicated, you acknowledge that, as with any business endeavor, there is an inherent risk of loss of capital and there is no guarantee that you will earn any money as a result of your participation in the Program. Neither we nor any of our partners or affiliates will be liable for any direct, indirect, consequential, special, exemplary or other damages that may result, including but not limited to economic loss, injury, illness or death. By your participation in this Program, you agree not to attempt to hold us liable for any of your decisions, actions or results, at any time, under any circumstance. By signing, you also acknowledge that you have represented to the Company that payment of your Program membership fees will not place a significant financial burden on you or your family.

Refund Policy: We allow a refund within 60 calendar days of your purchase date (signed herewith), only if all 4 of the personalized coaching sessions conducted with you during that initial 60 days have been completed with your full participation and homework turned in on time. If you have met this requirement for at least four coaching sessions prior to the end of 60 days, and are still dissatisfied, you may request a refund at that point. We also reserve the right to withhold 10% from your refund for administrative costs involved in processing and supporting your order. For my enrollment in the “Business Mastery Transformation” Program, with this payment to BB&B Corp, I, the participant, verify that I have read and agree to the terms and conditions detailed herewith. This instructional course is provided for information only and no representation or warranty of any kind regarding specific or general benefits, monetary or otherwise, was or has been made by Ted Miller III, any of his agents, or affiliated companies. I/We acknowledge that neither Ted Miller nor any of his staff or affiliates is responsible for my/our business success or failure, for my acts or omissions, or the appropriateness of my business decisions heretofore or hereafter. I also understand that I, as the participant, may cancel this agreement by midnight on the third business day after this signing. If I cancel within this three-day period, I will be promptly reimbursed any initial payment made, pending the return of any Company product. To cancel enrollment, I understand that within three business days from the date of this order (date next to my signature), I must email my request to StephanieVaughan@TedMiller3.com with my full name, specific Program purchased or ordered, and on what date. Prior to any refund given, all physical products given to you as part of the program must be returned in re-salable condition to: BB&B Corp, 515 NW Saltzman Road #725, Portland, Oregon 97229, United States of America.
